2012 CPT CODING UPDATES

ATTENTION    PAIN PROVIDERS

There were a couple of changes made to some of the CPT codes used for pain management procedures. These changes will take place for Date of Service 01/01/2012 and after.

1. Code 27096 for Sacroiliac Joint injections will include image guidance (fluoroscopy or CT) as of 01/01/2012. Please make sure to note what type of image guidance is being done with the injection.

2. Codes 64622 - 64627 for Radiofrequency Ablation of the Cervical, Thoracic, Lumbar and Sacral spine have been deleted and codes 64633 - 64636 have been added. Please note that for codes 64633 - 64636 image guidance (fluoroscopy or CT) is an inclusive component of these codes. Please make sure to note if you used image guidance and what kind when completing your APRs. Also note that the old codes 64622 - 64627 were for a single level of the spine and the new codes are for each facet joint. Please specify which level you were performing the RFA on and how many facet joints at each level the RFA was done on. Below are listed the old CPT codes and the new codes so that you can see the changes.

Old Spinal RFA Codes:

64622 Destruction by neurolytic agent, paravertebral facet joint nerve; lumbar or sacral, single level

64623 Destruction by neurolytic agent, paravertebral facet joint nerve; lumbar or sacral, each additional level (List separately in addition to code for primary procedure)

64626 Destruction by neurolytic agent, paravertebral facet joint nerve; cervical or thoracic, single level

64627 Destruction by neurolytic agent, paravertebral facet joint nerve; cervical or thoracic, each additional level (List separately in addition to code for primary procedure)

New Spinal RFA Codes:

64633 Destruction by neurolytic agent, paravertebral facet joint nerve(s) with imaging guidance (fluoroscopy or CT); cervical or thoracic, single facet joint

64634 Destruction by neurolytic agent, paraverebral facet joint nerve(s); with imaging guidance (fluoroscopy or CT); cervical or thoracic, each additional facet joint (List separately in addition to code for primary procedure)

64635 Destruction by neurolytic agent, paravertebral facet joint nerve(s); with imaging guidance (fluoroscopy or CT); lumbar or sacral, single facet joint
